In the Cyberpsyocho-quest where you have to find Hal Santos (and fight Anna Nox), there is a dead NPC that you can jack into (to weaken the Cyberspycho before your fight). I just found that after I had jacked in, I left the area and then came back to finish the fight. I could jack into the corpse again, but this time nothing popped up and I could't cancel it, so I was stuck and had to reload.
