This is not an epic intro, Is short and with poor graphic, but when i saw this for first time, I was totally impressed. Hope you enjoy it. Since this video doesn't fulfill requirements I wouldn't dare to enter. But I think, that this intro deserve to be mentioned :).

I remember liking the Soul Edge intro from PSX, think it was pretty epic at the time... ;p

My choice: The intro from Starcraft Expansion, Brood War.

Why this? The first thing you see is war. Humans fighting for survival of the race, and losing. You see them struggle, die, fear for their lives and only barely able to help themselves. You see the horrors of betrayal, the strong weaponizing creatures for their own gain, and the last horrible feeling of knowing that you've been abandoned by your own, the hopes of survival defeated as you're rushed upon by an army that wants but one thing, to taste your flesh between its jaws, unstopping, unceasing, the world growing cold until you're another forgotten casualty on a world left for dead.

i'm in. if you want a good laugh, you will not find a game with a more goofy intro (or theme song) than "rising zan: samurai gunman," a ps1 game from 1999. you play as the SUPER ULTRA SEXY HERO, zan / johnny, who is both a samurai and an "old west" cowboy. but before the ridiculous game itself begins, there are 3 back-to-back cut scenes that are absolutely priceless.
