What does it do? In other words, do I need to install that over the base diamond edition that comes with GOG, or do I only touch this if I'm having issues with the premium modules?

Note: I've never played Neverwinter nights in my life.

you have to install it after getting a key from support http://www.gog.com/support/neverwinter_nights_diamond_edition/online_play_in_neverwinter_nights_diamond by filling out the form in the link. this is for online play but is also good for some modules so it is a good idea to get it right away.
