Aye, sorry. I know there are two issues, and I just assumed Koschay was speaking of the bug you mention (number 1 in your post), as he wrote "then after a roll suddenly HERP DE DERP no Signs, no block, no items until I either get hit or swing the sword once." That sounded like the bug to me, but I apologize if I was mistaken. The bug needs to be fixed ASAP, I agree (it's killed me dozens of times). The balance issue with the animations and actions, while not a bug, also need to be looked at and hopefully tweaked to be more consistent, like you say. Both these problems have caused a lot of frustration for me while playing, and I'm just happy that I'm not playing on Insane, in which case my entire playthrough would have been destroyed because of the deaths that usually follow when this happens.

Occasionally with me Geralt freezes almost entirely, and it's always when my sword is out; I can't sheath or slash with my sword. I can move about though, and after a sign cast Geralt behaves normally again. This is almost always happens after I killed some enemies, not during combat.

Yea, same here. Seems to happen after I use bombs. It sucks.

Getting hit doesn't always solve the problem, either. Sometimes it takes multiple hits for the game to work properly again.

Also, sometimes after I throw a bomb and I can't attack, Geralt becomes temporarily invincible.

It is absolutely frustrating. It always happens at the worst times: intense combat situations.

At this point I am going to hold off on a third play-through and wait for a fix for this and maybe a few more free DLC releases.
You are describing the bug, a very real and concrete problem. I am describing the amount of recovery time between animations that CDPR has deemed appropriate.
