I really want the Madman achievment from steam, so i'm starting the game again in insane mode, i need some tips to reach it!

Did you play the game on hard mode and for several times? If not then... just don't even try :P

If you did than here are some tips: Quen lvl 2 + all of it's expansions. You should have alot of bombs and the best gear that's possible. You should go on "sword path", finishing moves, vitality and protectin are most important.
Go SAFE! Avoid fights as much as possible.
Go on Roche's path... I found it easier to get better gear by earning money from arm wrestling. Moreover, In Roche's path you have less fights.
Avoid doing side quests if they require you to fight... and the best tip i can give you is: be careful ^^

BTW: Before fighting bosses and doing tricky stuff you can load your save from the hard mode and fight those bosses several times till you'll be sure that you can beat him with closed eyes.

Im farming money in fistfight before doing some quests!
In Act I, take care not to go out of town from the second exit (I denote the first exit as the one beside Lobinden) or you might get swarmed by endregas. I once had 5 endregas chasing me and the narrow space restricts your manouevrability.
So, does insane difficulty not save at all? or does it save at certain areas so you dont have to play 40 hours with no saving?

You can save whenever you want but if you die once all your insane saves are being currupted and you can't load them anymore :\
abuse bombs/throwing knives as much as you can

pray you don't die during act 2 in a specific fight :(
protip: don't die :PPP ahahah I could not resist!
Yeah, I'd toss two levels into bombs & traps in the Alchemy tree.
Please keep us updated - I will be awed if you beat the game on your first playthrough on insane...

I do envy the level of immersion you'll have when Geralt loses a good deal of health... or when you walk in to some new dungeon for the first time and have no idea what dangers lurk around every corner...

... but ultimately I would have to recommend playing on hard your first playthrough. There are some fights that I imagine are going to be just... erm... insane ly difficult without knowing exactly what to do.

Your choice of course, and gl hf! :DDDDD
